ABSTRACTObjectiveTo explore the clinical value of ultrasound-measured snuffox radial artery resistance index (SBRI)combined with inflammatory markers forpredicting sepsis.MethodsAtotalof113 patientswith suspected sepsis admitted to our hospital were enrolled and divided into a sepsis group(n=51) and a non-sepsis group( n =62)according to the Sepsis-3.O diagnosticcriteria.SBRI was measured bybedsideultrasound.Procalcitonin(PCT),C-reactive protein(CRP), interleukin-1(IL-1),interleukin-8(IL-8),interleukin-1β(IL-1β),tumor necrosis factor- α (TNF- α ),matrix metalloproteinase -2 (204 (MMP-2),matrixmetalloproteinase -9 (MMP-9),white blood cell count(WBC),and neutrophil count(NEU)were determined usingabiochemicalanalyzer.Thediferencesinaboveparametersbetweenthe twogroupswerecompared.Apredictive model based on SBRIcombined withinflammatory markers was established byLogistic regresion analysis.Receiveroperating characteristic(ROC)curve wasdrawn toanalyzeitsdiagnostic performance inpredicting sepsis.ResultsTheSBRI,PCT,IL-1, IL- 1β ,TNF- α ,MMP-2,MMP-9,WBC,and NEU were higher in the sepsis group than those in the non-sepsis group,with statistically significant differences(all P<0.05 ).A predictive model based on SBRI combined with inflammatory markers was established by Logistic regression analysis:Logit (P) =-2.101+0.502 x SBRI+0.327×PCT+0.356×IL-1+0.031 × MMP-2.ROC curve analysis showedthattheareaunder thecurveforthecombinationof SBRIand inflammatorymarkers was0.773(95%CI:0.687-0.858),with a sensitivity of 82.0% and specificity of 91.0% .Conclusion The combination of SBRI and inflammatory markers can be used to predict sepsis,and demonstrates certain clinical value.
