Therapeutic Approach to Persistent and Refractory Allergic Rhinitis Based on the Theory of Chronic Disease Involving Collaterals

Abstract：Allergic rhinitis isachronic inflammatory conditionof thenasalmucosacharacterized by nasalpruritus， sneezing，nasal obstruction，and sweling of nasal mucosa.Traditional Chinese medicine（TCM） classifies it as biqiu （sniveling nose）.Patients with persistent and refractory allrgic rhinitis often exhibit features of collteral disease，characterized by inferior turbinate hypertrophy，periorbital hyperpigmentation（dark circles），and cyanotic lips，which align pathologically with the pathogenesis of stagnation and obstruction of collterals in the field of TCM.Therefore，the treatment of persistentand refractory allrgic rhinitis can be conducted from the perspective of chronic disease involving collaterals.The pathogenesis of persistent and refractory alergic rhnitis primarily stems from deficiency of the lung，spleen，and kidney organs，leading to phlegm and blood stasis accumulating inthecollaterals.Forthetreatmentofpersistentandrefractoryallergicrhinitis，collateralunblocking methodscan beusedonthebasis oforgan pattern differentiation，and therapyof reinforcingdeficiency and unblocking colaterals is usedas the fundamental therapeutic approach．Moreover，pungent-flavored collateral-unblocking agents are used as adjuncts，and blood-activatingand stasis-resolving herbsandcollateralunblocking insect-derived drugs are chosen if necessary，aiming to allviate nasal symptoms，enhance quality of life，and achieve superior clinical outcomes.
