Analysis a short-term prognosis prediction model for acute ischemic stroke after thrombolytic therapy based on multimodal magnetic resonance imaging and ASPECTS score Xing Anding, Xu Xuran,Fu Xiaoming. , ,Jiangsu ,China
【Abstract】 Objective To explore the eficacy combining multimodal magnetic resonance imaging (MRI) and ASPECTS (Alberta Stroke Program Early CT Score)in constructing a short-term prognosis prediction model for patients withacute ischemic stroke (AIS)after thrombolytic therapy.MethodsA total 116AIS patients treated inour hospital from January2023 to June 2024 were analyzedanddivided intoa goodprognosisgroup (n=65) and a poor prognosis group n=51 )according to short-term prognosis.The basic data,multiparameter magnetic resonance imaging resultsand ASPECTS scores the two groups were compared.The significantly diferent variableswere includedinthemultivariate Logisticregresion model,theshort-term prognosismodel was constructed,and thereceiver operator characteristic (ROC)curve was drawn toevaluate thediferentiationability the models.ResultsCompared with the good prognosis group,the poor prognosis group had a higher NIH strokescale(NIHSS) score,a higher proportion positive asymmetic prominent veins sigh(APVS),and a lower diffusion weighted imaging(DWI)-ASPECTS score (P<0.05) .Logistic regression analysis showed that NIHSS score, positive APVS,andDWI-ASPECTSscorewere independent influencing factors for the prognosis AIS patients( P (20 <0.05).The ROC curve showed that DWI-ASPECTS score had a high value in predicting poor short - term prognosis after thrombolytictherapy inAIS patients.The combinationNIHSSscoreand positive APVS could further improve the diagnostic value (Z=3.62, P<0.05 ).ConclusionThe short-term prognosis prediction model forAISafterthrombolytic therapy,basedonmultimodal MRIandASPECTSscore,exhibitsgood diagnostic value and is crucial for guiding clinical decision-making.
