膝关节腔内注射PRP联合间歇性低氧训练治疗早期膝骨关节炎的疗效及安全性

Abstract:ObjectiveToobserve theeficacyandsafetyof intra-articular injectionof platelet-rich plasma (PRP) combinedwith intermitent hypoxictraininginthetreatmentofearlykneeosteoarthritis (KOA).MethodsAtotalof 220 patients with early KOA were enrolled and equally divided into four groups using arandom number table.The low oxygen groupreceived intermitent hypoxic trainingalone,thesodium hyaluronate+lowoxygengroupreceived intra-articular injectionofsodium hyaluronatecombinedwith intermitenthypoxic training,thePRPgroupreceived intra-aticularinjection of PRP alone and the PRP+low oxygen group received intra-articular injection of PRPcombined with intermitent hypoxic training.Patientsofallgroupswere treated contiuouslyfor6weeks.The numerical pain rating scale (NPRS)score, inflammatory pain mediators [C-reactive protein (CRP),tumor necrosis facto1 -α (TNF -α ),interleukin-17 (IL-17),5- hydroxytryptamine ( 5 -HT) and substance P], oxidative stress indicators [superoxide dismutase (SOD),malondialdehyde (MDA)],the knee ijuryandosteoartritisoutcomecores (KOOS),timedupandgotest (TUG)results,ndadverseractions were compared between the four groups.ResultsAfter treatment,NPRS scores,CRP,TNF- α ,IL-17,5-HT,substance P and MDA levels were decreased inorder in the low oxygen group,the PRP group,the sodium hyaluronate+low oxygen group and thePRP+lowoxygen group.SOD levels,thescores for pain,knee-relatedqualityoflife,symptoms,exerciseand recreation,activitiesof dailyliving and total KOOSscores were increased inorder inthese groups,whilethe TUG time decreased in order (all P<0.05 ).There was no significant difference in the incidence of adverse reactions between the four groups (P>0.05 0.ConclusionApplying intra-articular injection of PRP combined with intermitent hypoxic training to treatearlyKOAcanefectivelyimprove theexpressonlevelsof inflammatorypainmediatorsandoxidativestresslevel, alleviate pain and enhance knee function and balance ability of patients,with high safety.

膝骨关节炎(KOA)属于退行性疾病,主要是关节软骨及关节滑膜出现增生、变性与损害所致,患者以膝骨关节疼痛及活动受限等表现为主,在老年人中具有较高发病率及致残率[1-2]。(剩余12553字)
