【Abstract】 ObjectiveTo explore the application effect of the fusion management mode in elderly patients with chronic diseases.MethodsA total of 10O elderly patients with chronic diseases admitted to our hospital from January 2O22 to December 2O23 were selectedanddivided into two groupsaccording to therandom numbertable method.Thecontrol group （5O cases） implemented routine management，and theobservation group （5O cases）adopted the management mode of medical and prevention integration on the basis of the control group， and both groups werecontinuously folowed up for6months.Theblood pressure，blood glucose，blood lipid levels， diseaseawareness，and qualityof life between the two groups were compared.ResultsThe levelsof blood pressure，bloodglucose，and blood lipidindicators in the observationgroup were lower than those in the control group after management，and the differences were statistically significant（ P <0.05）.The disease cognition scores of theobservation group were higher than those of thecontrol group after management，and the diference was statistically significant P <0.05）.The quality of life score of the observation group after management was higher than that of the control group，and the difference was statistically significant（ P <0.05）. Conclusion Intervening in theintegrated management model of medical prevention forelderlypatientswith chronicdiseasescan enhance theirdiseaseawarenessencourage their activeparticipationinself-management，and therebyimprove their blood pressure，blood glucose，and blood lipid levels，and enhance their quality of life.
