Study on correlation between SIR1, eGFR, SCr levels and the degree of coronary artery stenosis in middle-aged and elderly patients with coronary heart disease

YANG Jiemei, ZUMURETI ⋅⋅ Abudukeyimu,LI Jiao,WANG Haoyu, LI Xia (DepartmentofGeneralMedicine,theifthAfliatedHosptalofXinjiangMedicalUniversity,Urumqi83Oo,Xinjiang,China 【Abstract】ObjectiveTo investigate corelationbetween thesystemic inflammationresponse index(SIRI),estimated glomerular filtrationrate (eGFR),ndserum creatinine(SCr)levelsandthe severityofcoronaryartery stenosis in middleagedand elderlypatients withcoronaryatherosclerotic heart disease(CHD).MethodsAretrospective studywas conducted on114elderlypatients withCHDwhowereadmittedtotheFifthAfiliated Hospitalof Xinjiang Medical Universityfrom June to December 2O21.According to the median Gensiniscore,they were divided into mild stenosis group(groupA)and moderatetoseverestenosis group(groupB),with57patients ineach group.And then,theclinicalandcoronaryangiographic dataof thetwo groups were comparedandanalyzed.ResultsThe levelsof SCrandSIRI inthegroupBwere higher than those in the group A,while the eGFR level was lower,and the difference was statistically significant ( P <0.05).Multivariatelogisticregresionanalysis indicatedthatSIRIwasanindependentrisk factorformoderatetoseverecoronaryarterysteno sis in patients with CHD ( OR=1.594 , 95% CI:1.044 - 2.435, P=0.031 ).The Gensini score was positively correlated with SIRI and SCr,and negatively correlated with eGFR( P<0. 05 ).Thesensitivity,specificity and AUC of combined prediction of eGFR,SCr and SIRI for moderate-to-severe coronary artery stenosis in CHD patients were O.404,0.877,and 0.664 ,respectively(95%CI: 0.565-O.763).ConclusionSIRI,eGFR,and SCr areallsignificantlycorrelated with the degree ofcoronaryarterystenosis.SIRI isanindependentrisk factorformoderatetoseverecoronaryarterystenosis in middle-agedand elderlypatients withCHD.Thecombinationof SIRI,eGFR,and SCrlevels hashigh specificityfor predicting the degree of coronary artery stenosis.

冠状动脉粥样硬化性心脏病(coronaryheardisease,CHD,简称冠心病)是公认的危害人类健康的心血管疾病,随着我国人口老龄问题日益严重,流行病学调查显示我国CHD 发病率也逐年上升[1]评估CHD的严重程度主要依靠冠状动脉造影,而目前我国基层的医疗资源相对贫乏,因此寻找更为经济、简单、高效的实验室检查指标,对提升基层人群CHD的诊疗率和控制率具有重要意义。(剩余8403字)
