Abstract  Objective：To systematically search，evaluate and summarize the best evidence of home rehabilitation management for stroke patients，and to provide theoretical basis for medical staff to manage stroke patients at home rehabilitation.Methods：According to the "6S" evidence model，literature on home rehabilitation management of stroke patients in domestic and foreign databases，guidelines and professional association websites were systematically searched，including guidelines，evidence summary，clinical practice，systematic review and expert consensus.The search period was up to December 2023.Results：A total of 13 articles were included，including 2 clinical decisions，7 guidelines and 4 expert consensus，and 29 pieces of best evidence were summarized from 6 aspects of multidisciplinary team management，evaluation，planning and goals，home rehabilitation，quality control，feedback and supervision.Conclusion：The best evidence of home rehabilitation management for stroke patients provides an evidence-based basis for medical staff to carry out home rehabilitation management for stroke patients.In the application of evidence，the medical staff should consider the individual needs of the patient，take the patient as the center，and formulate a personalized home rehabilitation plan combined with the patient's own situation.
