ABSTRACTObjectiveTo explore the clinical value of snuffbox radial artery resistance index(SBRI)for guiding fluidresuscitation insepticshockchildrenbycriticalcareultrasound.MethodsAtotalof8septicshock childrenadmitted toourhospital wereselectedandrandomlydividedintoacontrol group(using criticalultrasound toguide hemodynamic management andfluidresuscitation)andanobservation group(using critical ultrasound combinedwith simultaneous SBRI monitoring togudehemodynamicmanagementandfluidresuscitation),with4Ocasesineachgroup.Thefluidintakeand positivefluidbalanceat1hafterfluidresuscitation,thenegativefluidbalanceandmechanicalventilationdurationat12hafter fluidresusiatio,ctateeaanetsat24d48teuiatiosellftiulstolic (LVEDV),left ventricular end-systolic volume(LVESV),maximum inferior vena cava diameter( IVCmax ),left ventricular outflow tract elocity-time integral(VTI),andSBRIat1hand12hafterresuscitationwerecompared between two groups.The28-day mortalitybetwenthetwo groups wascompared.ResultsTheobservation group exhibited significantly greaterfluidintake after 1hfluidresuscitationandalargerpositivefluid balancecompared tothecontrolgroup,withstatiticallsignificant differences(both Pmax and VTI were greater than those at 1hafterfluidresuscitation,whileLVEDVandLVESVweresmalerthanthoseat1hafterfluidresuscitation,withstatisticaly significant differences(all P max and VTI were greater than those in the control group,and LVEDV and LVESV were smaller than those in the control group,with statistically significant differences(all P
