Clinical value of ultrasound measurement of amniotic fluid sludge combined with cervical length and cervical anterior angle for predicting pregnancy outcomes
ABSTRACTObjectiveTo explore the clinical value of transvaginal ultrasound measurement of amnioticfluid sludge combined withcervical lengthandcervicalanteriorangleinpredicting pregnancyoutcomes.MethodsAtotalof386pregant women who underwent routineprenatal examinations inour hospital wereselectedand dividedinto twogroups basedon pregnancy outcomes:normal pregnancy group(247cases)and adverse pregnancy group(139 cases).Transvaginal ultrasound wasusedtomeasurethecervical lengthandcervical anteriorangle,andthepresenceof amnioticfluidsludgewasobserved. Thediferences inaboveresults betweenthetwo groupswerecompared.Logisticregresionanalysis wasused toscreen the independentinfluencing factors forpredicting adverse pregnancyoutcomes.Receiveroperating characteristic(ROC)curve was drawntoanalyzethediagnosticeficacyofthefactors individuallandincombinationforpredictingadversepregnancyoutcomes. ResultsThecervicallength inadverse pregnancygroup waslower thanthat inthe normal pregnancygroup,whilethecervical anterior angleand the proportionof positive amnioticfluidsludge werelargerthan thosein the normal pregnancy group,with statistically significant differences(all P<0.001 ).Logistic regression analysis showed that the cervical length,cervical anterior angle,and amniotic fluid sludge were independent risk factors for predicting adversepregnancyoutcomes (OR=2.511,2.632, 3.658,respectively,all P<0.05 ).ROC curve analysis showed that the area under the curve(AUC)of cervical length,cervical anteriorangleandamnioticfluid sludgeforpredictingadverse pregnancyoutcomes were0.793,O.81andO.712,respectively (all P <0.001).The AUC of the three parameters combined application was 0.881 ( 95% confidence interval: 0.815\~0.959, P<0.001), which was higher than that of each parameter(all P<0.05 ).The sensitivity and'specificity of the combined application were 85.61% and 88.67% ,respectively.ConclusionThe combination of amniotic fluid sludge,cervical length,and cervical anterior angle has a certain clinical value in predicting pregnancy outcomes.
