【Abstract】ObjectiveTo evaluate the safety and eficacy remimazolam combined with remifentanil for electronicbronchoscopyinfrailelderlypatients.MethodsA total58frail elderlypatients undergoing electronic bronchoscopyat People's from March 2023 to March 2024 were randomly divided into two groups: remazolam group (group R)and propol group (group P), with 29 cases in each group.Anesthesiainduction wasperformed with intravenous injection etomidateand pump-infusion remifentanil.The R group received intravenous injection remimazolam benzenesulfonate and pump-infusion for maintenance anesthesia,while the Pgroup received intravenous injection propol folowed bypump-infusion for maintenance anesthesia.A laryngeal mask was inserted once the sedation depth was achieved,and electronic bronchoscopy was initiated.The Mini-Mental State Examination (MMSE)score was assssed on the day before surgeryand before leaving the room.Blood pressure (BP),heart rate (HR),bispectral index(BIS)values,and SpO 2 (20 valueswererecordedatpre-induction(TO),laryngealmaskinsertion(T1),trachealscopeinsertion(T2),end bronchoscopy (T3),andpatientawakening (T4).Thetime awakening,orientationrecovery time,and the incidence intraoperative and postoperative adverse reactionswere recorded for both groups.ResultsThere were no statistically significant differences in general conditions,examination time,andMMSE scores between the two groups ( P >0.05).At T1,T2,and T3,the systolic blood pressure,diastolic blood pressure,heart rate,and BIS values in the Pgroup were lower than those inthe R group,with statistically significant differences ( P <0.05). The time to orientation awakeningand orientation recovery time in the Pgroup were longerthan in the R group ( P< 0.05).The incidence injection painand hypoxemia,hypotension,bradycardia,and respiratory depression in the P group was higher than in the R group (P<0.05) .Conclusion Remimazolam combined with remifentanil has goodsedation effects,stable hemodynamics,mild respiratory depression,rapid awakening,andno obvious impact oncognitive function in patients undergoing electronic bronchoscopy.It has fewer adverse reactionsand was safe and effective.
