意大利科学联盟立场声明解读：药物审查和处方精简

【Abstract】Multimorbidity oftenleads to polypharmacy，increasing theriskof drug interactionsandadverse drug reactions.Medicationreviewanddeprescribingare crucialstrategies forimprovingtherapeuticregimens，aiming toenhance patients'overallhealthandqualityoflifethrough reviewing andreducing inappopriate medicationuse.In March 2O24，the Italian ScientificConsortium issedaposition statement onmedicationreviewand deprescribing，outlining theirkeyelements， methods，available tools，and clinical value.Basedonananalysis of this statement，this article introduces the concepts andobjectives of medication review anddeprescribing.It elaborates on the main processes from four aspects：comprehensive medicationhistoryreview，medication therapyassessment，multidisciplinary medicationtherapyplanning，andfollow-up.The articlereviewsimplementatonoutcomesacross diferenthaltharesetings，analyeschallngesandcopingstrategiesinChina， andaims to provide theoretical supportfor promoting medication reviewand deprescribing byclinicians for their patients.
