肺癌化疗患者外周血单个核细胞中SITR1Nrf2基因表达与肺部感染的关系分析与研究

【Abstract】ObjectiveTo analyze the relationship between silent information regulator 1(SITR1) and nuclear factor erythroid-2-related factor2(Nrf2)genes inperipheral blood mononuclear cells (PBMC)and lung infection in patients undergoing lung cancer radiotherapy.MethodsA retrospective analysis was performed on theclinical data of 2OO patients undergoing lungcancer radiotherapy in the hospital between January 2O23and January 2O25.According to presence or absence of lung infection,they were divided into infection group n=100 ) and an non-infection group (n=100) .The expression levelsof SITR1 and Nrf2 in PBMC were detected by realtimefluorescent quantitativePCR(qRT-PCR).Thepredictivevalue of SITR1and Nrf2 genes for lung infection wasanalyzedbyreceiver operating characteristic(ROC) curves.Therisk factorsof lung infection in patients undergoing lung cancer radiotherapy were analyzed byLogistic regression analysis.ResultsThemain pathogens ininfection group included Klebsiella pneumonia (25.8%) ,Escherichia coli (16.1%) and Staphylococcus epidermidis (19.4% ).The expresson levels of SITR1 and Nrf2 genes in PBMC in infection group were significantly lower than those in non-infection group( P <0.05).The expression levels of SITR1 and Nrf2 genes were significantly decreased in patients with low partial pressure of blood oxygen (PaO2) , tumor metastasis and vascularinvasion (P<0.05) .AUC values of SITR1,Nrf2 and their combination for predicting prognosis of patients undergoing lung cancer radiotherapy were O.673,0.703 and O.821,respectively. Hypoalbuminemia ( OR=1.887 , P=0.046),diabetesmelitus (OR=1.958,P=0.031),usage time of antibioticsnot shorter than 14 d(OR=1.844,P= 0.047),radiotherapy time not shorter than 14 d (OR=1.923, P =0.029) and long-term usage of glucocorticoids (OR= 1.908,P=O.022)were independent influencing factors of lung infectioninpatients undergoing lung cancer radiotherapy.ConclusionThe expression levels of SITR1 and Nrf2 genes in PBMC are corelated with the occurenceand developmentof lung infection inpatientsundergoing lung cancerradiotherapy.Clinicaly,combined detection of the two can predict the occurrence of lung infection.
