Analysis of medication orders for total parenteral nutrition in adult inpatients in a hospital Zhang Zhiping， ZhangZhiqiZheng Xiaojun.，Shanxi 030001， China

【Abstract】ObjectiveTo analyze theapplication rationalityof the prescription of total parenteral nutrition （TPN） ina hospital，which providedareference basis for further optimizing thereview rulesof parenteral nutrition and standardizing theuse of total parenteral nutrition.MethodsThe TPN prescriptions in December 2023 from theRational Drug Use System were collcted，andthenutritional risk screning of prescriptionswasretrospectively analyzed.Therationalityoftheprescription ofthetotal nutrition admixturewas commentedon.Results According tothisstudy，theproportionofadult inpatientsreceiving TPNsupporttreatmentinthishospital who had undergone nutritional risk screening using nutritional risk screening 2OO2（NRS 2O02） was 38.1% ，and the averageNRS2002 score was4.O±1.3.Of the194prescriptions，122prescriptions wererational，witharational rateof 62.9% ，including2 prescriptionswith irrational ratio of glucose to lipid，47prescriptionswith irrational ratioof heat tonitrogen，2prescriptions with irational divalent cation concentration，16 prescriptions with irrationalaminoacid concentration，unreasonablealanyl-glutamine intotal aminoacids in48cases，unreasonable fish oil in total fatemulsion in5cases，andunreasonable total liquidvolumein6cases.ConclusionThe nutritionalrisk screening onadmission toa hospital shouldbestrengthened.Theprescriptionof totalnutrition admixtureinthishospitalisunreasonableincompatibilitystabilityandnutrientratio.Pharmacistsshouldgive ful playtotheir professional skilsandoptimize therulesfor thereviewof parenteral nutritionprescriptions.Nutrition doctors should strengthen educationand training to guide doctors to issue reasonableprescriptions，which will help the use of TPN.
