Efectsof tacrolimus combined with dapagliflozin on renal and immune function in patientswith IgA nephropathySu Xiaoxia.Department of Nephrology，Qinzhou Second People's Hospital，Guangxi 5350o，China

【Abstract】ObjectiveTo explore the effcts of the combination of tacrolimus and dapagliflozinon renal and immune function in patients with IgA nephropathy.MethodsA total of 70IgA nephropathy patients diagnosedand treatedinourhospital wereincludedinthisstudyfrom January 2O2O to October2O23andwere randomly divided intotwo groups.All patientswere givenroutine treatmentssuch asdiuresis，electrolyte correction，blood pressurereduction，anddietary guidance，aswell asoral administrationof prednisone acetate tablets.At the same time，the control group （35cases）was treated with dapagliflozin，while the observationgroup （35cases）was treated withacombinationof tacrolimusanddapagliflozin.Theclinical eficacy，renal function indicators （blood creatinine，urea nitrogen，24-hour urine protein level），immune function indicators （CD3 β+ ，CD 4+ ， CD8+，CD4+/CD8*），and adverse reactions were compared between the two groups.ResultsThe overall fficacy rate of the observation group surpassed the control group significantly （ P <0.05）.Following the intervention，markers pertaining torenal function，including blood creatinine，urea nitrogen，and24-hour urine protein levels，were notably reduced in the observation group compared to the control group （ P <0.05）. Regarding the indices of immune functionality，there was a marked elevation in the levels of CD 3+ ，CD4 k+ ，and the CD4+/CD8+ratio in the observationgroup compared to thecontrol group.Conversely，the CD8+levels weresignificantly lowerin the observational group （P<0.05） .Interms of the overall frequency of adverse reactions，the observationgroup exhibitedaslightlyhigherrate （11%） compared to the control group （9 % ），though this differencedid not reach statistical significance （P>0.05） .ConclusionThe combination of tacrolimus and dapagliflozin has ahigh therapeutic efecton IgA nephropathyandcan promoterenal functionrecovery，improves immune function，and issafe.
