ICU专科临床药师的药学服务实践

摘 要 本文介绍上海交通大学附属第六人民医院ICU专科临床药师的药学服务实践，药学服务的主要内容包括药学查房、药学会诊、药学监护、医嘱审核、不良反应监测、用药咨询等，在保障危重症患者临床用药安全、合理方面取得了一定的成效。

ABSTRACT The pharmaceutical care practice of clinical pharmacists in ICU of the Sixth People’s Hospital affiliated to Shanghai Jiao Tong University was introduced and the main contents of their service included pharmaceutical ward round， pharmaceutical consultation， pharmaceutical monitoring， medical orders examination， adverse reaction monitoring， drug consultation and so on. The pharmaceutical care in our hospital has achieved a certain effect in ensuring clinical safe and rational medication.
