医养结合机构开展老年人群“六养”健康管理新模式的实践探索

摘 要 本文梳理了老年人群健康管理的重要意义、面临的机遇与挑战及华东疗养院探索开展老年人群“六养”健康管理模式的实践情况，阐述了“六养”的主要服务内容、实践成效及未来展望等，为医养结合背景下养老服务体系建设提供思路。

ABSTRACT This paper summarizes the important significance， opportunities faced and challenges of elderly health management， and exploration of development of the health management mode of “six cares” for the elderly in East China Sanatorium， and elaborates on the main contents of the services， the effectiveness of the practice， and the outlook of the future for “six cares”， so as to provide the ideas for the construction of the medical and elderly care service system in the context of integration of medical and elderly care integration.
