应用PDCA循环管理降低我院住院药房退药率的效果

摘 要 目的：降低我院住院药房退药率，提升医药护团队服务效率。方法：通过住院药房医院信息系统（hospital information system， HIS）比较实施PDCA退药管理措施前后1年的退药原因和频次，以及年（月）退药率。结果：改治疗方案、药房无药和患者出院为退药主要原因；实施PDCA退药管理后，住院药房年退药率低于实施前，且随着PDCA循环增加，月退药率持续降低（P

Effect of the application of PDCA cycle management to reduce the rate of medication withdrawal in our inpatient pharmacy

ASBTRACT Objective： To reduce the medication withdrawal rate of our inpatient pharmacy and improve the service efficiency of the medical and nursing team. Methods： The hospital information system （HIS） of the inpatient pharmacy was used to compare the causes and frequency of drug withdrawal and the annual （monthly） withdrawal rate before and after the implementation of the PDCA withdrawal management measures for 1 year. Results： Changing the treatment plan， unavailability of the medicine in the pharmacy， and patient discharge were the main reasons for medication withdrawal. After the implementation of PDCA withdrawal management， the annual rate of medication withdrawal from the inpatient pharmacy was lower than that before， and the monthly rate of medication withdrawal continued to decrease with the increase of the PDCA cycle（P
