机器人辅助Wallace法回肠通道术治疗根治性膀胱切除术后输尿管梗阻的疗效分析

ABSTRACT：ObjectiveTo investigate the feasibilityand eficacyof robot-assisted Wallce ilealconduit inthe treatmentof ureteral bstruction folowing radical cystectomy.MethodsAretrospective analysis was conductedon the clinical dataof 8 patients with postoperative distal ureteral obstructionafterradical cystectomytreatedatthe FirstAfiliated Hospitalof Nanjing Medical University during Aug.2Ol8and Jun.2O24.Thecohort included7 males and1 female，aged 51-68 years （mean：58.8years）．Preoperativeimagingconfirmedtheobstruction siteand predicted itsetiology.Allpatientsunderwent robot-aisted Wallceilealconduitandwereregularlyfollowed postoperatively.Perioperativedata werestatisticallyanalyzed. ResultsAll 8procedures were sucesfully completed.The operation time ranged from 12oto 398 minutes（mean： 298.38 minutes），and intraoperative blood loss from 50 to 300mL （mean： 112.5mL ）.Postoperative complications occurred in 3cases according to the Clavien-Dindoclasification：one GradeIand two Grade II，with no major postoperative complications（Grade Il and above） observed. During a follow-up of 21-77 months，hydronephrosis and renal function showed varying degresof improvement.ConclusionRobot-assisted Wallace ilealconduit issafe，feasible，and efective in the treatment of ureteral obstruction following radical cystectomy.However，for malignant obstruction caused by tumor recurrence，combined comprehensive therapy is necessary to reduce recurrence risk.
