[Abstract]ObjectiveToexploretheclinicalcharacteristicsandmaternalandfetaloutcomesof pregnancyinalogeneicrenal transplatrecipients，andtoprovideatheoreticalbasisforperinatalmanagementoffemalerecipientsofallgeneicrenal transplatation.MethodsRetrospectiveinclusionof11alogeneickidneytransplantrecipientswhoterminatedtheirregnaciesin ourobstetricsdepartmentfrom January2o05toDecember 2022asstudysubjects.Theclinicaldataof pregnant women were colectedandfollwedup.Thetimingofpregnancy，managementofpregnancy，timingofterminationofpregnancy，modeof terminationof pregnancyandprognosisof motherandchildwereanalyzed.ResultsAtotalof11caseswereincluded，withan average gestational age of （ 28.9±3.1） years （ranged from 24 to 33 years）.The mean interval between pregnancy and kidney transplantation was （68.8±9.0） months （ranged from 15 to l14 months）.The main complications during pregnancy were urinary protein （9/11，81.8% ），anemia （7/11，63.6% ，hypertensive disorder complicating pregnancy （6/11，54.5% ），intrauterine growth restriction （4/11，36，4% ）and urinary tract infection （1/11，9.1%）. One patient was induced due to missed abortion，while the remaining 1O cases underwent cesarean section to terminate pregnancy.The average gestational age during delivery is （35.14±1.22） 0 weeks （ranged from 26.43 to 39.29 weeks）.The average birth weight of the newborns was （2270.00±260.89）g （ranged from 600g （204号 to3 150g ）.Themain neonatal complications included preterm birth （7/9，77.8% ），lowbirthweight （4/9，44.4% ），intraventricular hemorrhage （3/9，33.3% ），atrial septal defect （2/9，22.2%） ，necrotizing enterocolitis （1/9，11.1%） ，andanemia （1/9，11.1%）ConclusionThepregnancyofalogeneickidneytransplantrecipientsisahigh-riskpregnancywithmultiplematernalandfetal complications.Choosing theappropriatetimingof pregnancyand providingmultidisciplinarycollaborative managementsuchas obstetrics，kidney transplantation，and neonatology can achieve good maternal and neonatal outcomes.
