Li Fei’s Clinical Experience in Staged Treatment of Acute Ankle Sprain Based on the“Staged Protocol of Acupotomy，Acupuncture， andHerbalConsolidation"Model

Abstract：Acute ankle sprain，a common clinical injury of the musculoskeletal system，is characterized by sudden symptom onset and slow recovery，significantly impairing patients’motor function and quality of life. ProfessorLiFei posits that thecore pathogenesis of this condition involves contraction of the sinews and stagnation of qi andblood due to external trauma.The pathological mechanism generally evolves through three stages：“sinew node-inducedcollateral obstruction”，“lingering stasis retention”，and“deficiency complicated by stagnation”. Consequently，he advocates for staged treatment aimed at unblocking obstruction and restoring balance，activating colaterals and regulating equilibrium，and reinforcing theroot and sustaining stability.He has proposeda staged therapeutic strategyof“release in the initial stage，regulationandunblocking in the middle stage，and consolidation of therootin the final stage".Thisapproach entails initiallyusing acupotomy to release sinew nodes and break down adhesions； subsequently applying moving qi acupuncture combined with warm needling to regulate qi and blood and dispel stasis and turbidity；and finally employing herbal fumigationand immersion to warmly nourishand consolidate therapeutic effects.By integrating the essence of external treatments in traditional Chinese medicine，this modelconstructs acomprehensive intervention strategy embodying the principlesof“supplementation within dispersion”and“tranquility withinactivity”，offeringanovel perspective for the clinical diagnosis and treatment of acute ankle sprain.
