[Abstract]Objective：Toexplorethemodulatoryefectsofacupunctureonfunctionalconnectivity（FC）oftheanteriorinsular cortexipatientswithchronickneeosteoarthris（KOA）usingresting-statefMRI.Methods：Atotalof48KOApatientswere enrolledincluding26casesintheacupuncturegroupand22casesinthesham-acupuncturegroup.Allparticipantsuderwent resting-statefMRI scansandcompletedtheWestern OntarioandMcMasterUniversities Osteoarthritis Index（WOMAC）pain scalebeforeandafteracupuncturetreatment.FCchangesoftheanteriorinsularcortexandtheircorelationwithalterations in pain scores were analyzed.Results：After treatment，the WOMAC scores of both groups decreased（both P <0.05），and the acupuncturegroupexhibitedincreasedFCbetweentheleftanteriorinsularcortexandbilateralpostcentralgyrusaswel asrightprecentralgyrus，betweetherightanteriorinsularcortexandrightprecentralgyrusaswellasinferiorfrontal gyrus（FWE-corrected，voxel P <0.001，cluster P<0.05 ）.Moreover，the increase in FC between the left anterior insular cortex andrightprecentral gyruswaspositivelycorelatedwiththereductioninWOMACscore（r=0.414，P=0.O36）.Conclusions： Theacupuncture treatmentmodulatesFC betwen theanterior insularcortexandthesensorimotor network inKOA patients， sugestingthatthefunctionalreorganizationoftheanteriorinsularcortex-sensorimotorcortexpathwaymayserveasacitical therapeutictargetforchronicpainmanagement.Thesefindingsprovidenovelneurobiologicalinsightsintothemechanisms underlying acupuncture therapy.
