[Abstract]Objective:Toexplorethetherapeuticmechanismoftranscutaneousauricularvagusnervestimulation(taVNS)in thetreatmentofmildcognitiveimpairment(MCI)basedonresting-statefMRI.Methods:FortyMCIpatientswereincluded andrandomly dividedintothetaVNSgroupandthesham-taVNSgroup,with2Ocasesineach group.Bothgroupsreceived 24 weeksof taVNSorsham-taVNStreatment,respectively.Clinical scalescoresandfMRIdata were collctedbeforeandafter treatmentforbothgroups.Thediferencevaluesinclinicalscalescoresbeforeandaftertreatmentbetweenthetwogroups werecompared.Thebrainregionswithsignificantdiferences inregional homogeneity(ReHo)valuesaftertaVNS were comparedbetweenthetwogroups.Pearsoncorelationanalysiswasperformed between theReHodiferencevaluesandscores diferencevaluesinthetaVNSgroup.Results:After24weeksof treatment,theimprovementofMoCA-BscoreinthetaVNS group was better than that in the sham-taVNS group(t =-2.618,P=0.009 ).Compared with the sham-taVNS group,the taVNS groupproducedsignificantReHodecreasesintherightanteriorcingulatecortex(ACC)andleftinferiorfrontalgyrus,and significant ReHo increases in the bilateral cunei and leftlingual gyrus(GRF-corrcted,voxel P <0.01,cluster P <0.05).In thetaVNSgroup,theReHodiferencvalueoftherightACCbeforeandaftertaVNSwassignificantlypositivelycorrelated with the MoCA-B score difference value ( r=0.498,P=0.025 ),and the ReHo difference value of the left cuneus was negatively correlated with the MoCA-B score difference value( r=-0.510 ,P=0.022).Conclusion:TaVNS can improve the cognitive symptomsofMCIpatientsbyregulating theimportantnodesof thecognitivecontrolnetworkandvisual network.
