摘 要 目的：了解产程中常规采用阴道检查代替传统的肛门检查是否会增加产褥感染和新生儿感染，以及母婴并发症的发生率。方法：将研究对象随机分成对照组（肛门检查组）600例和实验组（阴道检查组）616例。采用单盲法观察产妇产后的体温、恶露、伤口愈合等产褥感染发生率，观察产妇产前产后的白细胞计数及分类，以及观察新生儿体温、新生儿感染等情况。记录在统一设计的表格上，按诊断标准判断产妇是否发生产褥感染，新生儿是否合并感染。结果：两组临床试验对比后发现产妇和新生儿的感染发生率无差异性，P＞0.05。结论：阴道检查，可有效地减轻产妇检查时的不适感，并且能直观清楚地了解产道情况，能准确地观察产程进展，而且又不会增加产妇和新生儿的感染率，是一种安全可行的检查方法。

AbstractObjective:Intention to probe into the fact whether it will increase the rate of lying-in women and babies infection and syndrome when traditional anus check is replaced by normal vagina check.Measure 1216 cases of lying-in women are randomly grouped into two,one is defined as a comparing group (Anus check group),and the other is defined as an experimental group (Vagina check group),600 and 616 cases.To observe the temperature,and cut cicatrisation,puerperal infection rate,leucocyte calculation and assorting before and after production of lying-in women,and observe newly-born babies'temperature,infection and upper respiratory tract infection.The uniform standard is to evaluate whether lying-in women are with puerperal infection,and whether the newly-born babies are infected also.Result Comparing the two groups after clinical experiments,we found lying-in women and newly-born babies'infection rate indistinguishableness，P＞0.05.Conclusion That,firstly pudendum are sterilized with 0.1% iodine,then pudendum are scrubbed with 0.5% iodine,and last vagina check is implemented with germfree gloves,can reduce the feeling of discomfort of lying-in women,and let doctors know clearly production organs,and observe the status of production precisely,while it will not increase the rate of lying-in women and newly-born babies infection, so vagina check is a safe and feasible way.

资料与方法
研究对象为2005年3月～2006年2月在我院妇产科住院分娩的产妇1216例，有下例情况之一者不纳入研究对象：①年龄≤18岁，≥45岁；②临产前1周使用过抗生素；③入院时体温异常；④血常规白细胞计数及分类异常；⑤在当地医院或诊所试产己进入活跃期后，转入我院妇产科的产妇；⑥有妊娠并发症、分娩并发症等；⑦胎膜早破超过48小时。（剩余796字）
